68 yobf with exquisitely tender subcutaneous nodules with overlying ecchymoses bilateral thighs (circumferential -stop abruptly at the groin and papliteal fossae) c-ANCA positive a@1:80 but PR3 and CPO negative. Remit urwith Prednisone 60 to 80 but return when decreased.
Hello and welcome to HCM, Tender subcutaneous nodules with overlying ecchymoses could be due to hematoma formation. Is there any history of prior trauma? Prior trauma can lead to soft tissue injury, bleeding and thus accumulation of blood within the soft tissues. C-ANCA positivity is suggestive of Wageners granulomatis and there is no association with subcutaneous nodule formation. Prednisolone is given for treatment of Wageners granulomatis. Thus, clinical assessment MRI of bilateral thighs is required to assess the location of the swelling and its extent.
