General & Family Physician 's  Response
Hi,

Acitrom should be stopped immediately as INR is way above the target range. For three days of skipping the dose the INR may be repeated if below 3-5, can be started at 1 mg per day. If INR is failed or achieve, or too low, dose can be incremented back to 2mg per day.

Hope I have answered your query. Let me know if I can assist you further. Wishing you a speedy recovery.

Regards,
Dr. Saddiq Ulabidin, General & Family Physician
