11 year old girl. whole brain images show small, microcystic foci or encephalomalacia in the bilater periatrial white matter. Patchy periventricular gliosis has improved when compared to previous studies. However there is progressive volume loss. What does this mean?
MRI findings suggests encephalomalacia/gliosis in white matter in bilateral parietal region of brain. Encephalomalacia is sequel of past insult to brain. It is a reparative mechanism in brain and brain insult may be in the form of ischemia, infection etc. It is important to screen all causes. As she is only 11 years old, metabolic disorder etc should be ruled out. There is improvement in the status of periventricular gliosis but there is progressive volume loss. Progressive volume loss is not a good sign and it may show ongoing insult to brain. So,girl need proper evaluation by a neurologist.
