1. The uterus is leaned forward over the
urinary bladder, its one of the normal variation of the uterine position you have nothing to worry.

Thin endometrium is abnormal, its not favourable condition for the fertilisation/
pregnancy,
irregular periods, post procedures this condition seen.
3.Intramural fibroid means presence of fibroid within the uterine muscles, and a part is seen in the sub mucous (inside).
4. A cyst ( a fluid filled ) structure in the ovary with benign features - which only needs to monitored, no intervention needed, unless very big and causing mass effect.

Cervical polyp - a growth in the cervical canal with a stem holding the growth, which needs attention - removal /biopsy suggested.
