with advancing age, our bone become weak and show signs of degeneration in the form of arthropathies; and to compensate that, the ligaments around the bones enlarge and thicken to prevent any injury. this is what's happening to your spine, the ligaments around the spinal canal have thickened and they are now compressing your spinal cord and nerves which may be the cause of your pain. since you already have had spinal surgeries, they are further aggravating your symptoms. surgical decision depends on how much pressure is there on your cord and how much pain you feel. in your report, i you didnt mention any damage to cord so i assume it is fine. and as far as your pain is concerned it is upto you if yu can bear the pain then you can go with medical management and if u cant bear the pain, then you should go for surgery, considering you other medical conditions as well.
