What Does This CT Coronary Angiogram Report Signify?

i am 56 years old; i had heart attack in 2008 and PCTA was done with two stents. I am also suffering from Diabetic and high BP. Last tuesday i had severe chest pain and immediately contacted a cardio. The stress test concluded with a report mildly positive for provocable myocardial ischemia. Earlier done CT coronary angiography reported the following: 1. LAD is type I vessel and show few eccentric calcific plaques in proxial segment showing 50 55% focal narrowing. A fair sized D1 shows subtle ostial soft plaque followed by dense concentric calcific plaques in mid D1 showing approximately 60-70% narrowing. In the RCA, a calcific plaque is noted in mid RCA between 2 separate stents showing approximately 50-55% narrowing.
