What Does This FNAC Test For Peritoneal Ascites Indicate?

Sir,My mom has peritonial ascites.. The CT finds out that Liver , pancreas , kidney & ovaries are normal .There diagnosis is that it is peritonial carcinosis. Then FNAC is done the result is that Mucin like material in the Cytoplasm S/O mallignancy. The doctor said that that it is cancer at stage 4.. without biopsy how does they take this decision. They cannot give answer that where the primary started.They answered that whether it is in intastane or in ovary. Please guide me

Hi,According to the information you have provided ,I will explain as follows. The test that you have described is called peritoneal fluid cytology. As per the report ,the pathologists suggest that there are abnormal cells in the fluid. These cells are most probably cancer cells since they are filled with mucin as per the report. There are many ways of detecting cancer, one of them is by examining tissues obtained by biopsy or after surgery. Another method is by looking for abnormal cells in fluids such as peritoneal fluid. Certain cancers of appendix/colon/ovary spread via the linings of the body cavity.Peritoneum is a type of lining. Once the cancer spreads into the body cavity, this is called Stage 4. This stage in medical language is called metastases. I presume that the pathologist has confirmed the diagnosis of cancer on the fluid cytology by appropriate special tests such as PAS/Alcian blue and Immunocytochemistry. Quite often the primary is difficult to detect.You may obtain the cytology slides and get a second opinion from another cytopathologist for confirmation.
