L5/S1 disc diffuse posterior bulge indents the traversing S1 nerve roots.Annular L4/5 disc bulge indents the thecal sac.facet hypertrophy at the L4/5 and L5/S1 levels. changes seen in the dorso-lumber spine most likely represents sequelae of an old Scheurmann's disease
There is degeneration of lumbar spine disc between L 5 and S 1. This is resulting in the disc herniating between the bones. There is also a tear in the annulus part of disc at L 4 - L 5. The nerve roots pass through these neural foramina and you might have severe nerve root compression at that level where the exiting nerve root is compressed at both levels.
The nerve pain may be controlled with restricted physical activity and pain relief medication. Please do not do heavy physical activity till you are comfortable. Surgery is not always indicated with small disc bulges but pain control measures must be taken. There are medications given for neurogenic pain and it works in most patients. You may also apply muscle relaxants and pain relief ointments.
Scheuermann’s disease refers to an abnormality in the growth of the vertebrae, where the front of the vertebrae does not grow as quickly as the back leading to wedging of the vertebrae. This usually occurs in the dorsal spine (the upper back) and may have varying degrees of severity.
