Hi,

I would like to note any history of alcohol intake if any and quantity and frequency of the same. GGT levels and grade 1 fatty liver may be attributed to alcohol intake. However, if there is no intake of alcohol then the same may be attributed to Non alcoholic fatty liver disease. The raised levels of AST and ALT also suggest the same.

Considering the increased levels of liver enzymes we must proceed with following:
1. CBC/ESR/LFT with Albumin levels/KFT/RBS
2. USG whole abdomen
3. Fasting Serum lipid profile
4. Thyroid profile(F3/FT4/TSH)
5. Fibroscan of liver(very important investigation)

We may start with the following treatment
1. Lifestyle modification is the mainstay with avoidance of fatty and high carbohydrate meals and increasing intake of antioxidants.
2. Daily brisk walking of 30 minutes duration 5 days in a week.
3. Tab Udiliv 300 mg for five days and repeat LFT again to target ALT and AST below 40 IU
4. Further treatment would depend upon the investigations as suggested

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Santosh K. Pandey, General & Family Physician
