Want to suggest switching to same drug for long acting & rescue. What is equiv dosage for Hydromorphone ER and rescue hydromorphone. What about use of Exalgo for ER?
Exalgo is an extended release formulation of hydromorphone hydrochloride.
Hydromorphone ideally should be used with patient-controlled analgesia (PCA) by a microprocessor-controlled infusion device with a lockout period after each demand dose, to prevent overdosing.
