Hello and welcome to HCM,
Your history suggests that you have had surgery for breast cancer for which the lump is removed.
The lymph nodes are negative for tumor metastasis and the margins are also negative.
Chemotherapy is not given once the lymph nodes turn out to be negative.
Radiotherapy is given when the margins are positive or when there is evidence of metastasis to distant sites.
Thus, a regular follow up with your oncologist is required.

The second problem that you are facing is that of enlarged heart and emphysema.
Enlarged heart can occur in heart failure or cardiomyopathy.
Emphysema is a condition wherein the lungs are enlarged. Enlargement of the lungs can be secondary to cardiac enlargement.

These two conditions appear to be unrelated to your breast cancer problem.

An assessment of the cardiovascular ad respiratory system is required to know the status of heart and lungs.
Thus, get the investigations done as ordered by your doctor.

Thanks and take care
