Menstrual pain is caused by normal uterine muscle contractions. When you hormones all line up to cause your period (even if there is no bleeding), you uterus produces a substance called prostaglandins. Prostraglandins stimulate your uterine muscle to tighten and relax - usually to help get out the blood and uterine lining that is due to shed at this time in your cycle. This is why Ibuprofen can be helpful. If you start taking 24-48 hours prior to when you usually bleed, or usually have cramps, if can stop or decrease the amount of prostaglandin which then stops or decreases your cramps.
