What Is The Medication For Normalising The Blood Pressure?

My husband (otherwise very healthy) had mitral valve repair at Clevelend Clinic in August for ruptured cords and flail valve. Repair was done robotically and he recovered well except that he is unable to tolerate a beta blocker and continued to have a resting HR of 105-115. Most recently, after being on Zebeta 2.5 mg for about one month he developed extremely low BP and chills (no fever or other evidence of a veg). His HR is now 120-126 upon awakening (comensurately low BP) and later decreases to the mid 90s as the day progresses. The cardiologist is going to start him on 120 mg of diltiazem. My question is: should he wait to see if he adjusts to the d/c d beta blocker and will the calcium chanel blocker be effective to equalize his BP and HR. He completed 32 weeks of cardiac rehab.

Hello! Thank you for your question! Regarding your concern I would like to know what kind of procedure is performed for mitral valve repair (and associated mitral regurgitation)? Was it a Mitraclip? Can you give concrete data on valve functionality after the procedure? Does the valve exerts important residual mitral regurgitation? Does it exist a high diastolic transmitral gradient? What is the actual left ventricle EF? The answers for the above questions are found in the ECHO report. They would help to elucidate reasons of tachycardia. If important residual problems have remained then neither Bisoprolol, nor Diltiazem could be much effective to control his resting heart rate and at the same time to preserve blood pressure.
I think that we have to check even for additional triggering factors. A complete blood count, thyroid function tests would be useful.
