Why Is My Lower Back Puffy And Sore?

The lower part of my back is puffy, it is quite sore and I have back ache, I ve been to see a doctor, and he said it was just fat. Now, I m not a doctor but a bit of fat at the lower part of your back should not ache and hurt. I have been fatter then I am now. Could you help if possible.
