Brief Answer:
No alcohol and smoking are not safe

Detailed Answer:
Hello XXXX,
You have not described about your stroke in detail but as you were admitted and treated at a good hospital diagnosis must be stroke.
Now you have young onset stroke with hyperhomocysteinemia and clot In left ventricle.
Spontaneous clot may be due to cardiac rhythm or structural disease or hypercoagulation disorders. As you have been investigated thoroughly for cardiac causes the other possibility is more.
You should get coagulation profile also.
As far as treatment you should take drugs regularly and maintain you INR between 1.5 to 2.5
You are at high risk of recurrent stroke so I will advise to have healthy lifestyle, exercise, diet.
You must not consume alcohol as it changes the drug metabolism and levels. Additionally smoking increases the atherosclerosis and will be bad for your health.
There is no safe levels of both these factors and better to refrain from them.
Hope you recover early and be healthy.
