Can An Epigastric Hernia Cause Acid Reflux?

Question: Hello Doctor
I just want the answer on different symptoms:
I have been diagnosed with a hiatal herna 2cm and NERD; which is reflux even when it's not acid.
I also have this pressure feeling under my breastbone, which I think is the hiatus hernia symptoms. Some think it's not clear because normally hiatal hernia that small don't give symptoms.
After making a ph-value test and manometry Ã…rhus university hospital wants to offer me a fundoplication for curing my symptoms. they said I have reflux but it's not acid and a small hiatal hernia. I take 40mg nexium and have symptoms daily.
I have had esopagitis a few times, Grade B and A.

Here comes my question:
I have always have a little bulb a little bit lower than my symptoms area of my hiatus hernia; and as far as I have google it; I can see it's a small Epigastric hernia. The fat bulb is very small and I have never had pain in the area. I just want to know; do you think the pressure feeling is from hiatus hernia or do you think this epigastric can have causing my symptoms all along ? ..

The pressure feeling often come after drinking coffee and can also come after a very hot or long shower; and it can also have reflux following.

I have attached pictures that show the area of where I feel the pressue up agains my chest or outwards it's hard to describe. I have had this for 6 years; but I can remember having the Epigastric all my life. ( the picture where I don't point the area, there you can see the small bulb from epigastric hernia )

To answer these with reasoning wherever necessary:
Small epigastric hernia, which is present there all your life and has not increased till now nor caused any direct symptoms may not be associated with your symptoms.
Secondly, increase in pressure after drinking coffee can not be related to epigastric hernia and there is also reflux following.
Please check if the small bulb you have increases or decreases on coughing and whether there is tenderness on pressure. This will never cause the reflux.

On the other hand, could this be symptoms because of the 2cm hiatus hernia I have ? or maybe the hernia is a bigger than they see on endoscopy.

This is something I have had for 7 years and it comes and goes but is more chronic definitely.

1. The pressure feeling I have under my breastbone is something irritating and sometimes when symptoms are bad; I have harder to breath with my diaphragm(like breathing with your stomach).
2. nausea
3. it raise my puls and get my heart to beat harder. ( I have done ECG and heart-check, and there is nothing there ) ..
4. reflux
5. hard to belch
6. relief when I belch
7. worsen if I eat fast and too much
8. Sometimes it's gone totally gone; and don't come back for 2-6 days.
9. Comes back suddenly; after being in a hot shower, eat to much, drink coffee and can last for weeks.
10. When it's pressing up agains my breatbone; it gives relief if I lay down and massage my stomach ( like hiatal hernia massage ) ..
11. Diaphram spasm ( under-breastbone ) solar plexus spasm; like the stomach is squeezing for 2-3 seconds. Comes and goes.
12. Reflux can give me pain in my back.
