1. Your acid reflux has probably been building up for months to years before your severe attack.
2. The discomfort in the cheat and throat pain may be due to Nexium (esomeprazole). Dryness of the throat and non specific chest and abdominal pain and discomfort are known side effect of esomeprazole. Ask your physician to change the medicine to rabeprazole, dexrabeprazole or pantoprazole. Domperidone may be added. Many weeks of therapy are required to control acid reflux along with dietary and lifestyle changes.
3. Loss of appetite is also a known side effect of Nexium. You may have to go easy on the quantity of the richer foods (common on catering menus) as you grow older.
4. You may share an endoscopic video of your throat, larynx, esophagus and stomach for further advice.
5. A normal person also has acid reflux 4-5 times a day but does not notice it, People who have more than 15-20 episodes of reflux on a day suffer the most. In addition to gastric acid, digestive enzymes and bile may reach the throat, worsening the burning in the throat. These enzymes may remain in the throat and get reactivated when the pH falls with the next reflux, damaging the delicate throat tissue faster.
6. Get a USG Abdomen done to rule out a hiatus hernia. H. pylori infection needs to be ruled out. Esophageal manometry and swallowing studies may be done.

I forgot to tell you, I have had 3 bad sorethroat and sinus infections in the last 5 weeks. Been on amoxicillin a week each sorethroat espisode. 500 mg twice a day.

1. Your symptoms of cloudiness, depression, chest and stomach discomfort may be a side effect of Nexium and other proton pump inhibitor drugs. Prevacid and Prilosec are also drugs of the same class. If burning in the chest is not a common symptom, you may try Ranitidine (H2 receptor antagonist) instead.
2. Pain referred to the back indicates esophageal (food pipe) irritation or esophageal spasm. Try some antacid or Mucaine gel for a few days.
3. It is difficult to predict future attacks, however they will be lesser in intensity with the control measures followed religiously. Sleeping regularly with five pillows will result in neck pain and cervical spondylosis.
4. A Post Nasal drip (PND) due to sinusitis will increase gastritis and acid reflux.
5. Multiple courses of antibiotics will also cause gastritis and worsen acidity. Take a probiotic and multi-vitamins to lessen these side effects.

1. The sensation of a muscle lump in the throat is known as 'Globus Pharyngeus'.
2. Inflammation of the throat, voice box or food pipe causes spasm of some of the numerous the muscles attached to these structures. There are many other causes for this 'Globus Sensation' such as cricophayngeal spasm (the muscle guarding the opening of the food pipe), esophageal motility disorders, stress, cervical spondylosis or ossification of the hyoid, laryngeal cartilages with age and so on.

5. No tablet is expected to be lodged for a very long time as they are designed to disintegrate.

1. Laryngo Pharyngeal Reflux (LPR), as mentioned above, is known as silent reflux.
2. Changing the PPI will not prevent the side effects. You have to change to a different class of drugs such as H2 receptor antagonists like Rantitidine (Zantac).
3. You may have to take regular medication for at least 8-10 weeks before you get lasting relief.
4. This is a common problem and is managed initially with medications and later by diet and lifestyle modifications. Surgery is a limited option in cases such as hiatus hernia (fundoplication) or paraesophageal hernia. You will need an Abdominal USG (Ultrasound) to rule this out.

1. If Zantac is working fine for you, you need not worry about going back on a PPI.
2. Both PPIs and H2 Receptor antagonists reduce acid production in the stomach but by different mechanisms.
3. You may also be able to take a lower dose of the PPI or avoid it completely if it does not suit you.
4. You will feel better in time. Add diet control and lifestyle changes. Do no eat any food that worsens your acidity. Eat at least two to four hours before sleeping.

1. You may take Gas X (Simethicone) as it has no side effects. Peppermint oil in softules is useful. Peppermint liquid drops get inactivated by stomach acid.
