Thanks for posting query.
From history it appears that there might be having some injury or soreness on frenulam resulting in infection giving rise to red spot and tenderness.
Frenulam is a part where foreskin of gland is adheres to shaft of penis.
Foreskin can not be pulled back above this site due to adhesion.
Many a time after forceful sex or masturbation this skin is injured and having main vascular supply at this point there is profuse bleeding and inflamed vessels.
Go for one antibiotic and anti-inflammatory medicine course for 3-5 days after consulting your doctor.
Apply antibiotic cream locally.
Clean glans properly to remove smegma with warm water.
He should avoid sex or masturbation for few days.
