How Can Constant Vomiting In A Child Be Treated?

Question: hi Doctor,
My son is 18 months Old,
1) he started getting vomiting from past 1 week, started on Saturday last week.. mostly if we give some food like Milk or ragi ganji he was vomiting.
2) after 1-2 days he got fever around 101 as well, Bit of Cold
we went to Pediatrician , he suggested few machines like Vomikind, Mefanorm plus( for fever) ,salvincold AF.
3) Fever was controlled in 2 days.. he was bit ok for 1-2 days.
4) Motion is normal.
Now Major question is at times he still 'Vomits ' Intermittently every alternate day..
looks like mostly with Milk and when we try to give some other food..
Not sure if food is not getting digest or is it normal to vomit once a day?

please advise,.. is it normal to vomit once a day Or intermittently ?
your suggestions would be appreciated.

following are details,
also, he looks bit low and bit rest less..

1. Did the vomiting increase after giving Mefanorm Plus?
Not really, he was ok for 1-2 days. fever was also reduced

2. Is there any weight loss or no gain in weight for the past 3 months?
we feel its same weight, however he looks he might have lost weight.. not sure.

3. Is the vomiting only once a day or more than that?
mostly Once per day.. at times twice.

4. Does he arch his spine back during sleep and extend his neck?
Not really, however he was doing arch spine while sleeping 1-2 months back.

5. What is his birth weight and length?
3.6 pounds

6. What is his current weight and length?
weight:10.7 KG,79 CM

7. What is his exact date of birth?
5th Feb,2016

Your kid is normal and this occassional vomiting need not be worried about

Detailed Answer:
Hi...I have gone through your answers thoroughly. I have calculated his height and weight centiles according to his date of birth.

Both of them are at 20th to 25th centiles and it is normal.

In percentile calculation - any value in-between 5th to 95th is normal.

So the concept is that when the height and weight parameters are normal, we need not worry about the occasional vomiting.

But from next time please avoid Mefenamic acid for fever.

For fever - Paracetamol can be given in the dose of 15mg/kg/dose (maximum ceiling dose of 500mg) every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

Mefanorm Plus contains Mefenamic acid which is a NSAID. It carries the risk of gastritis and renal problems in the long run. Please do not use Mefenamic for fever.

Thank you Doctor, Does this mean vomit once a day is normal?
If yes, how do we avoid ? any home remedies ?
also, one more question : his stomach is bit bigger then normal for 4-5 months..he is also having bit of hemoglobin defecency for which we did a course for 2 months.

please advise any suggestions for Vomit

Hi... I understand your concern. But as the weight and height are normal - which means the growth is normal and it doesn't require treatment.

1. Once a day vomiting is normal.

2. You can avoid this by avoiding over feeding and stopping the feed once he starts to refuse while he is being fed.

3. Distended stomach is to be worried about only if the doctor palpates it and finds any organomegaly or distended liver or spleen. Otherwise it may be only fat. You need not worry about it.

4. You say that he had iron deficiency 2 months back. Can you please upload the images if you have any of the lab tests done then. How low was the hemoglobin and what about other parameters like - MCH, MCV and MCHC (in the report)?

Suggestions on iron deficiency and home remedies for vomiting

Detailed Answer:
Hi...I have seen the reports. It is not a major iron deficiency. The normal value in this age group is 11 and it was 9.8.

You need to use iron supplement for 3 months and not 2 months.

I suggest for his current weight you need to use -

HEMSI-PD drops (30mg/ml) 1.2ml once a day 15 minutes before breakfast for 3 months.

I have already assured you that regarding the stomach distension - you need to worry only if the doctor say there is swelling in liver and spleen. It will be known by the doctor only after touching and seeing it. If you are really worried about abdominal distension I suggest you consult your pediatrician regarding this confirmation please.

Home remedies include ginger and lemonade. But these may not be palatable for a 18th month old.
