Brief Answer:
Personal opinion: No; Professional opinion: Yes, to prolong life

Detailed Answer:
Hello once again XXXXXXX ma'am.

Thank you for writing to us.

I understand you are writing for your father. His chances of survival with or without chemo are bleak for longer than 1-2 years, and so it is my personal opinion to avoid the chemo. This will give him a little more comfort than with the chemo, but professionally I would recommend the chemo to prolong life.

But personally, I always believe in quality of life and not quantity. You should know that without the chemo he will live half or less of the expected life span chemo can provide him with, but it will be higher in quality than the life he will live with the chemo. And so, without the chemo we can expect a few months (~6 months approximately).

I hope you find my response both helpful and informative. My best wishes and prayers are with sir. Please do not hesitate to write back to me for any further information, I am always here to help.

Best wishes.

Brief Answer:
Chemo can only slow down the progression at this stage, not cure it

Detailed Answer:
Hi ma'am.

Stage IV is the final stage and so chemo can only do so much in slowing down the progression of the cancer, but it cannot completely eliminate it. By the time it destroys a few cancer cells, there are numerous others being produced.

Appetite loss with colon cancer, especially stage IV is common, and unfortunately there is nothing we can really do about it. In one patient I had a similar experience with, he was a young 37 year old male, who gradually stopped feeding, and we had to feed him through a tube during the last few days/weeks of his life.

Please do not hesitate to keep in touch, I am always here to help.

Best wishes.

Appetite only after chemo has messed up. So if we do procedure on liver like TACE can that increase survival even more ?

Brief Answer:
I only hope for his best and nothing else, so my honest opinion is:

Detailed Answer:
Hi ma'am.

Procedures like TACE are used for liver cancer that has metastasized from colon cancer, can help to improve liver health mildly, but how good it would be on improving the survival rate cannot be predicted.

The reason for this is that by the time we treat the liver, cancer cells from the colon can settle elsewhere again and result in another metastases. Either way ma'am, with all the procedures and chemo we can prolong life, but the quality of life will be bad and it is your personal choice to go ahead with these procedures after which he may live slightly longer, but will always be suffering.

Best wishes.

Ok is oxyplatin and raltitrexed a good Combo?

U see his cea was 65 before chemo . After 1 round it jumped to
80 and after 2 it's now 55. What can this mean ?
doctor
Answered by Dr. Shoaib Khan (1 hour later)
Brief Answer:
Yes it is a good combo ma'am and should be effective

Detailed Answer:
Hi XXXXXXX

Yes, the combo is good and should be effective.

The initial rise in CEA was the expected rise without the chemo and that just shows that the chemo had not begun its action, and the second round shows how the chemo is actually helping.

I think the chemo will help to prolong life by as less as a few months and maximum by a few years (1-3).

Best wishes.

Should
We proceed with Germany the immune boosters and TACE?
doctor
Answered by Dr. Shoaib Khan (59 minutes later)
Brief Answer:
Only if complete physical has been provided

Detailed Answer:
Hi ma'am.

If the chemo is ongoing, then you can resume with Germany the immune boosters and TACE (as long as this has been recommended by his doctors) and also if a complete physical approval has been granted by his personal physician overtaking his general health decisions.

Best wishes ma'am.
