Hi and thanks for the query,

Azithromycin is well indicated for acute infections. Generally, within 48 to 72hours, remarkable changes should be observed.

It is generally prescribed with drugs to reduce inflammation and fever like anti inflammatory drugs (like Ibuprofen) and pain killers like Acetaminophen.

In case no remarkable change is observed even with normal doses and having taken the drug correctly, it might mean that the infection is too severe and might require intravenous antibiotics or that another antibiotic be prescribed, since infection might not fall within the spectrum of action of Azithromycin. The germ could also be resistant to Azithromycin.

If within 48 to 72 hours no remarkable change is seen, you might need a review from your doctor.
