Question: How do you make differential diagnosis in MRI image between brain hematoma due to TBI(7month post brain surgery) and excess salt (2900mg)intake thru peg tube if brain damage occured.

Thanks for your query.

Normally there would not be a confusion between these two as they have entirely different pictures on MRI. Hematoma usually would occur as a space occupying lesion, while rapid rise in sodium (in a patient with originally low sodium) presents as more diffuse dehydration like changes in the brain, and specifically myelinolysis (dissolution of nerve fibers) in the pontine area.

Of course there is change in the brain

In both of these conditions there will be change in the brain. In high sodium, there may or not be changes depending on the severity but blood in brain will definitely be seen on MRI.

Thank you. I have asked other doctors friend and family what happened. Everybody said nothing serious. Now I am getting closed what I thought when I looked at mri disk.Is there any treatment on brain shrinkage due to hypernatremia? It has been 7 months. I knew something going on. But I was not sure. She has been vision problem(Homonimous heminepsia). 3rd nerve palsy others. At this point I do not know what to do? Please help me.

They said it was ok 2nd Mri but 3rdmri shown differently.. Widen subdural space.

Another question is that ~ 2900mg sodium could cause damage enough. Thank you.

diffuse shrinkage is not only due to high sodium but also other causes like brain atrophy, age-related or radiation induced.

Hx. 65old, f,TBI rt subdural hematoma, Sx removal of hematoma, icu, 2 cans of Cambodia chicken noodle soup +5 cans of regular liquid food(total 2900mg sodium)thru peg feeding tube in March. I will get MRI report soon. Mri image different after sodium diet.

very unlikely for this much sodium to do anything

This is just the sodium through food and very unlikely to do brain damage and produce the symptoms you mentioned. They are probably due to the primary disease, hematoma long term-effects and post radiotherapy long term effects.
