The blood creatinine is 40 umol/L ( normal 40-85).

The creatinine is near the lower limit of normal. It is by no way deranged. The importance of a single creatinine value must be interpreted in view of the patient's muscle mass. If the muscle mass is low then the creatinine which is its byproduct would also be on the lower side. Liver disease, aging and protein malnutrition can also lead to a low creatinine level. Workup can be done to rule out liver disease and protein malnutrition. If they are normal then no further investigation is required.
