Is Hysterectomy The Only Solution For Endometrium Hyperplasia?

Question: I am 50 yrs with history of excessive bleeding,usg showed raised endometrial thickness,gone through hysteroscopic d&c , pap smear normal, histopathology comfirmed Hyperplasia without Atoypia, what to do,either go for hystrectomy as adviced or for progesterone iud advised by some other doctor

Thanks for writing.
Endometrium hyperplasia without atypia had only 1% risk of endometrium cancer. As you have undergone complete evaluation and other tests are normal it would be more beneficial to have a progesterone Iud as there is no need for invasive surgery.
You have to understand that this is not a permanent solution unlike hysterectomy which Is a one time procedure
But in my clinical experience I have seen many cases easily resolved with either progesterone iud or oral progesterone.
Thus need for unnecessary surgery is decreased.
Considering all the facts I would recommend progesterone iud.
The final decision has to be taken by you as both as methods of treatment for endometrium hyperplasia.

undergone hysteroscopic d&c on 3/5/14.bleeding stopped on11/5/14 , again spotting on 15/5/14 stopped on 23/5/14.can i go for mirena implant this month or wait for one cycle

Thanks for writing back.
You can go for mirena this cycle. There are no reasons why you should postpone
mirena can be inserted even if there is some spotting. There will be on and off spotting even after mirena insertion for some time. After few weeks the mirena will cause delayed and decreases bleeding and decrease the endometrium hyperplasia.
