I understand your concern.
But I am afraid that till date, surgery is the best option to treat an established case of anal fistula.
There are different types of treatment for various types of fistula, depending on location, depth of involvement and presence of abscess etc.
However, surgical treatments in form of seton techniques, fistulotomy and modern surgical methods like video assisted fistula treatment etc are very effective. The post operative relief from symptoms is quite effective as well as recurrence rate is also low.
Some non-surgical options like injection of fibrin glue in the fistula tract has been tried widely but though immediate effect is good, the recurrence rate is high. Hence, its use is bit controversial.
So my suggestion will be to visit a General surgeon and get clinically examined. Accordingly, your treatment options can be decided. Surgical management should be the priority.

In case not operated what problems occur .she has been taking antibiotics when ever puss and pain is there .can she continue like this. In case we decide to go for surgery can you suggest the best surgeon in XXXXXXX

Hello.
1.In case of not going for operations, complications like flaring up of pain, persistent discharge and infection might occur.
2.Yes she can continue like this if the problem is not severe. She can continue with antibiotics in cases of abscess and discharge etc. I would also suggest maintaining anal hygiene, taking sitz bath regularly to decrease pain and application of local anesthetic jellies if required.
3.I, not being from XXXXXXX wont be able to comment much on it. You can consult doctors from XXXXXXX who will be able to give you some specific information about it.
