Brief Answer:
Explained in detail

Detailed Answer:
Hello and welcome

Your concern is appreciated

Most of the perineal symptoms are attributed by general public to hemmorroids. The most common presentation of hemmorroids is bleeding from rectum, pain, pruritis or prolapse these symptoms can be present in number of anorectal diseases as these symptoms are very non specific.
Digital rectal examination and proctoscopic examination should be done thoroughly.
You need to visit general sugeon for that.
Low fiber diet, constipation or diarrhea, prolonged sitting, heavy lifting, obesity, pregnancy are relevant in history.
The presence of reducibility of prolapse if present will help in grading of internal hemorroids and further treatment.
Other condition that needs to be differentiated from it are inflammatory bowl disease it often co exist with hemorrhoids,
Varicosities from portal hypertension are distinct from hemorrhoids, acute proctitis, condyloma acuminata, rectal prolapse, polyps, and colorectal cancer which is very rare at this age.
The gastroenterologist consulation is necessary when we are suspecting upper gastrointestinal bleed like from esophageal varices or varicosties due to portal hypertion in the rectum for which upper GI endoscopy and evaluation for other stigmata of portal hypertension is done.
The managment of hemmorroids specially actively bleeding is surgical
There are many options which include cryotherapy
Sclerotherapy
Surgery called hemorrhoidectomy
which has increased recovery time and post surgical pain but low reccurance rate
other is stappled hemorrhoidectomy
Which has reduced post surgical pain and recovery time but more recurrance rate
Hemorrhoid artery ligation,
Rubber band ligation is another treatment option
Photocaogulation and liquid nitrogen treatment are other treatment options for bleeding hemmorroids
So you need examination by a general surgeon who will then asses the grade and examine the hemmorroids and will decided how it should be managed effectively.
The life style modification and diet modification like increase fiber diet relieve constipation to decrease straining at stools etc.

Hope it helps

Thanks
