Brief Answer:
As discussed in details below.

Detailed Answer:
Hi.
Thanks for your query about your brother.
Noted the history of bite- ?insect made left wrist swelling - for three days - subsided with skin flaking - but has soreness at right arm and right upper back preventing sleep - applied tiger balm patches on back, blue emu on arm with ace bandage wrap ...

The problem looks to be an insect bite with quite an unique poison affecting locally but at the same time can pass proximally through veins and/or lymphatics causing the problem as you have mentioned.
Hence I would advise the following in such a situation:
Consult a Doctor for clinical evaluation to know the exact cause, to know the soreness at arm and upper back is due to spread of the poison or developing cellulitis so that the most proper treatment can be instituted
Please do not apply anything locally other than Liquid povidone iodine to keep the area sterile.
This is very important as the nature and the source of the poison and the aftermath are not known for sure and delay can have further complications.

I hope this answer helps, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
