Question: The problem I have is that I can go 7-10 days without having a bowel movement and then when it decides to come, it comes with full force. Heavy cramps, nothing produced the first time or two to the bathroom that day, then gut-wrenching diarrhea all day. Then, back to nothing for 7-10 more days. I eat very clean and healthy, which includes an apple each day.

You have not mentioned since when you are having these symptoms.

I feel these symptoms are probably side effects of the medication you are taking.

You are severely constipated with probable fecal impation. This is resulting in symptoms of alternating constipation and diarrhea.

If colonoscopy was not done before, I would advise you to get it done to rule out obstruction.
If it does not show anything significant, then colon transit study, blood glucose and thyroid function tests should be done.

Meanwhile you can take miralax for relief.
