This is not cancer. Any cancer takes several years to develop and the fact that you never had any trace of cancer from the last colonoscopy and normal PSA means cancer is very unlikely at this time. The most common cause of pain in the anal area is due to straining especially with hard stools or small wounds. I will recommend you monitor only now to see whether it will resolve spontaneously or need more attention.
If your stool is of hard consistency, I will recommend that you eat much vegetables, fruits, do exercise often and drink much fluids. This way, your stool will become softer and relief the pains.
