I was a fair skinned XXXXXXX 39 years old male. I have dark pigmentation in my face and neck area. This happened in april 2015 and its more than 7 months now. Some doctors are saying its melasma..solar melanosma.. Some say its lichen planus pigmentosis. Some say its lichen planus actinis.. When this started i had slight fever , heavy headache, dry mouth and joint pain. After doing blood test it was found out that my RA factor is very high nearly 245 but ANA negative... Is this because of rheumatoid arthritis or any infection..i am now using hydroquinone cream, elica and acretin and sunscreen when i go out in the sun. But not much improvements. How long will this take to go completely. Do i need to do any further investigations ?

I have gone through your query and I have also reviewed the Images.
-Were you using any cosmetics on face prior to this problem?e.g any fairness cream, oils etc Or
-Any oral drugs/ medications like antibiotics, antiinflammatory medications, which you used for headache, fever?
This seems like a diffuse pigmentation unlike melasma. I will suggest a possibility of either lichen planus pigmentosus Or phototoxic dermatitis. Both these pigmentations are deeper and are slow to respond to treatment.
I would definitely suggest regular use sunscreen. Topically you may continue using these creams mixed together in equal proportions (1:1) and applied once daily at night.
You should see a noticable improvemnt of being on this regime for at least 4-6 weeks.

Thanks for the prompt response doctor. I had a facial done . After that i didnt use any sunscreen as i dont have the habit of putting sunscreen. I had vitamin D deficiency and out of my ignorance I used to go out in the sun to get some vitamin D at 10 am - 11 am... Slowly my skin turned darker day by day..

A product or facial which would normally be well tolerated may cause a photoallergic or phototoxic dermatitis in presence of sun's UV A Or UV B rays. I think this is what seems to have happened with you. A lot of such cases happen to common fairness products like fair and lovely, facials etc. You may follow the treatment as follows. The pigmentation would go down gradually.

Thanks doctor. What is the treatment i should follow? Hydroxycholoroquinone 4%, Elica and acretin mixed in small equal quantity in the night ?
Do i need to take any vitamin supplements or antioxidants? Can u suggest some medicine. I forgot to update ..my igE (Allergy test) was high..vitamin. B12 was normal... TSH and t3 t4 normal but thyroid antibodies were positive. Does it have any relation .
Will my skin go back to my normal color. Please find attached the photo of my face one year back..
