Suggest Remedy For Typhoid Fever

Question: My 13 year old son is got diagnosed by our doctor for typhoid. His test for Typhidot igm test came positive. he has been having fever since last 8 days, earlier the test came negative. Now the doctor has prescribed him Topcef 200 1.5 tabs BD. is this treatment correct. my son is having 99.4-100 F fever now as well. is this normal. How long it will take for him to recover completely. he still complains of occasional stomach pain.

IgM positivity for typhoid indicates an acute infection of typhoid. It may take at least 5-6 days to turn positive and probably that is the reason that it initially came negative. Topcef is a right medicine to be given and your doctor has done the right thing in prescribing it. Please continue it for 7 days. If I had a case like this, I would perhaps prescribed Zanocin (ofloxacin) 200 mg 1 tablet twice daily for 7 days alongwith. You may talk to your doctor about it whether he feels it necessary to be added.

Typhoid is known to cause high temperature continuously present for 7-10 days. Even after 8 days having elapsed, it may still take a week or more for him to recover fully but the temperature should be gone in the next few days. You would have to be patient till then. Give paracetamol to bring down the temperature as and when required. Meftal tablet can be given if the temperature does not come down even with paracetamol.

Thank You Doctor for providing the detailed response. couple of follow up question:
a) is intermittent stomach pain common while recovering from Typhoid?
b) His fever since today morning is fine, however we noticed that the fever reaches 100 (in armpit) around 5-6 PM in the evening but with Dolo 250 it comes down very fast to normal, is this normal too?
c) our doctor has suggested to him normal home diet and no special diet, is this ok or should we take care of any particular diet?

Thanks for asking back.

1. Intermittent abdominal pain could occur in typhoid especially because it is a gastrointestinal infection. I would not worry about it. But it could be also due to hyperacidity caused due to the medicines like dolo. In that case, giving the medicines in full stomach, and giving one capsule of omeprazole 20 mg once a day before breakfast should be a better idea.
2. Fever could occasionally rise higher and get relieved on medication. It is normal.
3. Normal diet is fine to be given. I would not suggest any specific modification. Just keep it low on fats, oils and spices; a suggestion for all.
