First of all, we need to confirm that these are metastases from carcinoid. Because relapse after 15-20 years is bit uncommon. To confirm, we should get serum chromogranin, 24 hr urinary 5HIAA and octreoscan or dotanoc-PET scan. Finally, a biopsy from any of the lesions will be required. The stage depends upon whether this has really spread or not. In case it has, stage becomes 4.
Usually in case of stage 4 disease removal is not done unless the lesions are causing any problems. Medicines are the key here.
If the heart lesion is causing problems then it should be removed, that would give the biopsy also.
Now, even if there is relapse and metastasis, this is a very slow growing tumor and long term survival is common. Octreotide (somatostatin analog) is usually given to control the disease and can be administered before cardiac surgery also.
