Suggest Treatment For Abdominal Bloating And Pain In A Person With Hypothyroidism

Question: I have been having upper right quad dull pain for sometime accompanied with gas and bloating. I have had an ultrasound of the abdomen which revealed nothing. My mother died at age 52 of cancer, had metes but they felt it was colon. Paternal grandparents died of stomach cancer, father healthy age 94. I also have hypothyroidism and take medication for that and prozac for depression/anxiety. One sister had breast cancer, a other sister and one brother heathy. I also have skin issues and still get hot flashes and allergies.

Thank you for the query.

I do not want to tell that you may have a cancer. Its too early for that. But as your mother died of cancer in such young age (if it was colon cancer, she must have it for 10 years), you are at higher risk of this cancer for sure.
Breast cancer of your sister, makes you at the higher risk of this cancer as well.

From the other hand, upper right abdominal pain can be caused by stomach issues including an ulcer, inflammation.

Ultrasound is less than sketchy test in case of stomach or large intestine diseases. In my opinion, you should have colonoscopy done as the first test (it should be done as soon as possible). In a meanwhile, blood work should be also checked (low hemoglobin level can indicate either stomach or large intestine issue).

Please dont wait with it. Especially that it may aggravate your anxiety. Once again, I do not tell that you have a cancer. I just want you to be sure you dont have it.

I have has several colonoscopies, all were good. I want to get an ERCP. Will that tell if I have an ulcer? I have test results I could attach, blood work, US, Lung CT. Would that help? I am a EMT. (Volunteer for 10 years) and a medical records medical malpractice defense paralegal. Is there another email address to respond to where I can attach those records?

ERCP can diagnose an ulcer.

Detailed Answer:
If you attach your last blood work and colonoscopy, it would be easier to help you. ERCP starts as a normal gastroscopy. So it can revel ulcer for sure. Do you have elevated liver enzymes or bilirubin so they want you to have ERCP?

There is no indication to perform ERCP at this point.

I have gone through your reports. Generally I do not see any indication to perform ERCP. So this is very strange idea. Please note that this test is invasive and rarely can cause serious complications.
Gastroscopy would be much more suitable at this point. Before any ERCP, abdominal CT should be done (if we think about any pancreas changes). Its way too early for ERCP.
