Suggest Treatment For Bone Marrow Suppression

Question: My father (age 83 years) is suffering from CKD and Ischemic Heart Disease. He was admitted in hospital in Feb'16 for treatment of Angina. During the process, it was noticed that his Hemoglobin level was very low (5.5). Further investigations (Electrophoresis and Bone Marrow Aspiration) revealed that he is suffering from Hypoplastic Anemia.

Presently, he is being administered Weepox Injections (4 K units) twice a week and has to undergo Blood Transfusion every 15-20 days.

What are the treatment options available for a person of this age suffering from Bone Marrow Suppression/ failure ? Are Anabolic Steroids safe for a person of this age (83 years) and medical history of Heart Disease, Chronic Kidney Disease ?

I have gone through your question. I can understand your concern.
You are suffering from hypoplastic anemia.

Your bone marrow is not producing enough red blood cells so you need external support by blood transfusion.

At age of 83 with chronic kidney disease low erythropoitin may be the cause of anemia. So treatment is either erythropoitin or blood transfusion.

Erythropoitin stimulate the RBC production so it will help to increase the hemoglobin.

But if it can not maintain your hemoglobin then external blood transfusion is needed.

Erythropoitin is costly and safe, blood transfusion is cheaper but risky.

So you can choose accordingly. Few other treatment options are there but you should go for bone marrow biopsy once.

Moreover please send me your other reports like complete blood count and other routine investigations.

So I can help you further to guide you. Right now you should continue with your injection. It is first line of treatment.

Hi, dear
I have gone through your all reports.
You have pancytopenia. Your RBC lineage is more affected so anemia is more significant with low WBC count and low platelet count.

You should go for current treatment and wait for some time to check the bone marrow response.

If you develop severe pancytopenia then ATG (anti thymocyte globulin) with or without cyclosporin or steroids can be given. Other supportive treatment like erythropoitin, GCSF, blood transfusion can be used according to need.

This all drug are prescription based medicine and need clinical examination and judgement before use. So consult your hematologist and discuss about all options.
