Brief Answer:
Get Semen culture done and take appropriate antibiotic.

Detailed Answer:
Hello
Thanks for your query ,based on the facts that you have posted it appears that you are known to have Chronic Prostatitis since 20 years and getting relapses of the same .

To be true it is really a cumbersome disease for the patient and treating physician .
One has to repeat the Semen culture every time after getting relapse of infection and take appropriate antibiotic as per culture report .

In Chronic Prostatitis I advise patients to do gentle prostate massage everyday .It helps to reduce the congestion of prostate and thereby reduces the severity of infection .

If you are sexually active have a sex thrice a week or ejaculate by masturbation .

Start taking either Doxicycline or Bactrim D,S along with anti inflammatory medicine Diclofenac twice daily and switch on to appropriate antibiotic as per culture report .

Hope I have answered your query ,please feel free to ask if you have more questions , I shall be happy to help you .
Thanks and Regards.

Brief Answer:
Repeat Semen Culture and take antibiotics if required

Detailed Answer:
Hello
Thanks for follow up .
Chronic prostatitis is characterized by repeated episodes of infection and the possibility of bacterial infection has to be ruled out by doing semen culture and take antibiotics ( If turns out to be positive for bacterial infection ).

Consult your Urologist and explore his opinion and advice regarding daily prostate massage as a modality of treatment.I am routinely practicing this modality in my patients of Chronic Prostatitis since last 45 years of my practice and getting encouraging results .

Thanks and Regards.
