Welcome to HCM
as I can understand from your information,
1) you are having headache that begins with empty stomach and relieves on taking pain killers.

here we need to consider condition such as migraine, regarding which I may require anwers on following questions
1) is the pain episodic?
2) when does it start? other precipitating factors besides empty stomach?
3) How long does the episode last?
4) does it begin with more noise levels, increased illuminants, on eating certain food stuffs?
5) is there a history of similar complaints and your relatives or parents?
6) is it associated with vomitting?

answer to these can help me to have a better clinical picture.

Brief Answer:
migraine is suspected

Detailed Answer:
Thank you for the information.
As i can understand here that there are certain triggering factors such as exposure to intense illuminance, head position and stress levels.
There is positive family history of the similar condition.
Presence of nausea.
responds to pain killers.
No organic pathology detected on head scan.

These factors and information is highly suggestive of migraine like condition.
It has no causative pathology and can occur in any individual, tends to run in family and can subside on itself.
Preventive measures such as avoiding exposure to triggering factors and use of over the counter medications for relief is the mainstay of treatment.
Considering all this my suggestions to you would be
Identify the possible triggering factors. make a list of those and avoid it. It could be exposure to light, noise, or injection of certain food stuffs like chocolate, etc. Avoid keeping stomach empty for long, avoid prolonged computer use.
Judicious use of over the counter medications like ibuprofen, etc can be used. In your case s there is history of good response to medication i would suggest to continue taking it on as an when required basis but mot more than thrice a day. Note: certain pain killers can cause gastric upset if taken frequently.
We have further option of medications if there is poor control with plain analgesics.
Lastly i would suggest you to see a local psychiatrist, as a detailed work up and examination can never be substituted by an online opinion. Moreover, certain drugs warrant prescription that can only be obtained from a doctor in person.
An Opthalmologic opinion for refractory errors can be considered as well.

Hope to have helped you and satisfied you query.
I wish you a good health and welcome for furthers suggestions.
Regards,
Dr Pankaj
