Question:
Deat doctor
I have a herniated disc at my L5-S1 and i have to do surgery in order to cut of the herniated part. What is the best method ( e.g. Endoscopic Discectomy... ) for this surgery ? Brief Answer:
Need to see your MRI report.
Detailed Answer:
Dear Sir,
Thank you for posting your query on HCM.
Well looking into your problem I would like to tell you that I would first like to see the Imaging report to determine the need for the performing the surgery . Getting spinal surgery at this age needs a strong reason and indication for the same . Herniated disc is not enough reason it there are no red flags .So first I would like to know the history and have a look at relevant reports to comment.
Take care.

Dear XXXXXX,
Thanks for reverting back.
The pics that you have uploaded are not very clear but still I can see the disc bulge between L5 S1 and it looks significant .
Please send me the reporting that radiologist have done for this film .
Second please mention all the symptoms that you are getting like shooting pains or any bladder bowel involvement etc.
Looking at the MRI I think surgery would be required but need to see the radiologist report .

Exactly, i have shooting pains sometimes during the day, and the pain is sensed in the left leg (external side of thigh). This pain occurs without any particular position of the body. I wanted to ask which type of surgery should i do? Which is the best ? Less pain, less recovery...
Here i also attach the report of raiologyd

Dear Sir,

See surgery is recommended only when there is some neurological deficit like numbness in the legs , bladder bowel involvement etc.
For pain surgery is not recommended . It gradually settles over time . other methods like epidural anesthetic block can be used for pain removal .
Your radiology report shows indentation of thecal sac but the extent is not mentioned. Still if your doctor feels that surgery needs to be done then endoscopic removal will be the best because only single disc involvement is there .
The radiologist should have mentioned the amount of indentation then it would have been easy to judge .
Can you tell me what is your occupation and how did you get this lesion, was it due to some injury because the choice of treatment will also depend on that also.

Thanks for your response.
Actually i had some numbness in my left side of my left thigh 1 year ago. But after 3 months , it had gone . 1 year ago i didn't know that i have disc herniation and supposed that this numbness was something to consider.

If the symptoms have been subsided already why do you want to get the surgery done . Surgery is quite risky thing and you should avoid it if you are not getting any symptoms . Thats what I think about it plus since one year has already been elapsed whats the fun of doing it now.
