Suggest Treatment For High Blood Sugar Levels

Question: I have my fasting sugar 130-135 and PP 180-190. At present I'm taking metformin 500mg twice a day. But still these sugar levels are the same . I'm non-smoker, non-alkohilic and vegetarian. Do I need to take some other mediciene or continue with Metformin 500mg. I play bedmintion daily for about 45 mints.

I understand your concerns on diabetic management.

Our aims in diabetic treatent are

1. Control diabetes
Fasting sugar less than 130 mg/dl, after food < 180 mg/dl and HbA1C < 6.5% for your age

2. Prevention of complications
Do an annual screening for diabetic retinopathy, neuropathy and nephropathy (dilated eye examination, vibration perception tests, urine micro albumin and renal function tests are usually done). Have this done every year after consulting a diabetologist or endocrinologist.

Your blood sugars are slightly higher than target. Previous HbA1C is 6.1 % (from your question in sept 2014)- that is also in target. So you can continue with metformin current dose. If your sugars are going up you may need to be started on additional agents.

If I were your treating physician the next drug I will start you on is gliptins. You can discuss about this with your doctor in next health visit. This group of drugs are with minimum side effects and comfortable to use.

Thanks a lot Dr. for your answer. As you have suggested for higher glocose levels i have to take Gliptins . What should be its dosage in a day and what should be the time to take it.

Thanks for the question.

There are different drugs in gliptin group. The dose depends on the type of gliptin.
This include sitagliptin (like Januvia 100mg once daily), Vildagliptin (like Galvus 50 mg twice daily), Linagliptin (Trajenta 5 mg once daily). Please consult the doctor before starting the medications. All medications are equally effective in controlling the sugars
