Question: if a macular hole has been there for over 8 months should it be repaired. Patient is 65 years old and is 5feet 6inch

I am Dr. DADAPEER K, an Ophthalmologist and I answer health problems related to eye.

It seems from the history that 65 year old is having macular hole for 8 months.

The surgical management is indicated in macular holes which are stage II and above.
The stage I macular holes include partial thickness macular holes and surgery is not indicated in them as more than half of them will undergo spontaneous resolution with improvement of vision.
The stage II and above macular holes include full thickness macular holes and surgery is indicated in them. Best surgical results are obtained if they are operated within 6 months. However after 8 months also surgery can be tried with fair amount of success rate.
The visual improvement occurs following surgery even in cases with long standing macular holes of more than 1 year duration. The success rate of surgery is in the rage of 50%-60%. Since it is 8 months following diagnosis surgery is indicated in this case if the stage of macular hole is stage II or more.
