I have gone through your query and understand your concern. You are mainly having meniscus injury. It will require most probably arthroscopic meniscectomy (surgery).Till then you may continue with quadriceps excercises and analgesic such as ibuprofen for pain relief. As you also has ACL laxity so some kind of knee splintage can be used to give rest to joint. Your second MRI also suspected fracture, whether you got recent injury to your joint?

Thank you for your response. I have never had a knee problem. My knee problem started this past summer with the gradual onset of pain. I have not had any kind of joint injury. At some point between the two cortisone shots, I felt something "pop" in my knee while I was stretching. I'm guessing that might have been when my meniscus was torn. My real concern is my insufficiency fracture or osteonecrosis. I have no idea how or why this happened or how it should be treated. Do you have any thoughts on this?
May need core decompression.

So you have two problems one is meniscus tear and another is osteonecrosis leading to insufficiency fracture. Cause of osteonecrosis is generally not known ( idiopathic). Popping is due to meniscus tear. Pain can be due to combined effect of both. The treatment of osteonecrosis depends upon its stage, and I feel this is probably it is in early stage. It can be treated by arthroscopy along with meniscectomy by decompressing the condyle (core decompression) by multiple drill holes ).
