Question: I have mild sciatica from spinal stenosis, but it has increased a great deal. My physio said my exercises will take care of the increased pain, but I have gotten worse. Could I need a hip transplant?? I am in good health and have always been very active until now.

I am sorry to hear that you are having so much pain with your sciatica and spinal stenosis. You have been participating in physical therapy but you are not improving. There are several options still before any surgery would be contemplated. you still have the option for chiropractic care, steroid injections and nerve blocks. If these do not help and the pain is intolerable then I would recommend surgery. The surgery would not be a hip transplant. They do not do hip transplants. You would need surgery on your spine which could be a laminectomy where the cut the portion of the disk that is impinging on your nerve or a spinal fusion where they fuse the bottom few vertebra together. That surgery will be determined by the type of spinal stenosis you have.

In summary
1. continue physical therapy
2. Try chiropractor therapy
3. Steroid epidural injections
4. Nerve blocks
5. Surgery laminetomy or fusion.
