Long term intake of gutka in which tobacco is the ingredient causes keratinization of oral mucosa which causes the patch. It also causes fibrosis below the layer of mucosa lining of oral cavity known as "Oral Submucous Fibrosis".
Submucous fibrosis causes reduction in size of opening of mouth and also painful opening of mouth.
This condition is unlikely to respond to Oral antibiotic like cefixime and Ofloxacin.
I would suggest you to consult ENT Surgeon for clinical assessment. Diagnosis depends on good clinical examination.
The treatment of patients with oral submucous fibrosis depends on the degree of clinical involvement. If the disease is detected at a very early stage, cessation of the habit is sufficient. Most patients with oral submucous fibrosis present with moderate-to-severe disease. Moderate-to-severe oral submucous fibrosis is irreversible. Medical treatment is symptomatic and predominantly aimed at improving mouth movements. Treatment strategies include the following
1. Steroids
2. Lycopene supplements
3. Pentoxiphylline
Absolute and complete cessation of intake of tobacco in any form is very important.
