An underlying infection could be responsible.

Thanks for asking on HealthcareMagic.
I have gone through the description. It seems that the breast and nipple are inflamed due to some reason. There could be an underlying abscess which is responsible for the inflammation. An abscess in other words is a localized infection. The resultant inflammation can cause the area to be sensitive and swollen. I would suggest some analgesic like naproxen taken as needed, and hot compress to be given alternating with cold compress. The infection needs to be contained and antibiotics might also be needed. If these steps provides relief, its fine, else, you should seek surgical assistance and get it re-evaluated. If there is an underlying abscess, it might need to be drained.
