I have this pain for months, and it does go into forearm and sometimes wrists and fingers.
No recent trauma
no pain when getting up in the mornings
pain is both at rest and activity, but does happen (usually) in the evenings.
I have a lot of headaches. On meds for prevention of migraines.

Thank you for getting back with the answers. I'm sorry for the delay. I have gone through the answers. Two most common causes that can result in pain includes muscular spasm & presence of cervical rib. i would suggest you to once get x-ray thoracic inlet done. it will rule out cervical rib. In that case following measures will help you in smooth recovery:
- naproxen twice a day for 5 days.
- take hot water in a rubber bottle and put it at the site of pain. It will give soothing effect.
- avoid lifting heavy weight till you become healthy again.
- ask some family member to do oil massage on your shoulder. It will give soothing effect.
Hopefully you will observe improvement soon but If pain persists then visit hospital for workup & physiotherapy.
