I have gone through your MRI report.The causes of these white matter hyperintensities may be-
1)Normal physiological in some patients.
2)Migraine may have associated white matter abnormalities.
3)Vasculitis
4)Hypertension
5)Demyelinating lesions like multiple sclerosis(MS)
6)Dementia

Your clinical picture does not appear to be fitting into Dementia or MS.
You can discuss with your treating doctor to get you screened for high blood pressure and vasculitis.
Meanwhile,continue treatment of your headache considering it as a primary headache(not related to these lesions).
Get a follow-up MRI done after 6 months.
