For decreasing the fever and chills, you can continue to use Tylenol. If you have no problems with excess stomach acid, gastritis, reflux/GERD, you can use ibuprofen instead (Advil, Motrin) but take after you have some food in your stomach.
But I am concerned that you have had this fever for 4 days. Given your medical history (thank you for providing this), I think you should go in to the urgent care or see your doctor today if possible. There they can listen to your lungs, etc. I think it would be a good idea, given the duration of your fevers and your history, to get a chest X-ray to check for pneumonia.
This may be a virus, such as influenza (although that usually gives a cough too) but it's best to get this checked out now.
Stay hydrated and try to get as much rest as you can.
One caution: If you go into the urgent care or doctor's office, make a mental note to not touch your eyes or nose until you have thoroughly washed your hands or used alcohol gel.
