Suggest Treatment For Pre Diabetes

Question: my c-peptide level is 14.7 and insulin level is 141, thyroid are normal and sugar level fasting 100 sugar level after 2 hrs of food 89 and HGA1 is 5.6. I am always feeling hungry after 2 hrs. and I had gain 6 kgs. in last 3-4 months. please advice the cause and treatment. Presently I am taking Indral 20mg 1-1 (for anxiety and palpitation) and Olmesartn 20 mg 0-1 (for BP). My LIPID profile is under acceptable limit

A fasting blood glucose value of 100 mg/dl is compatible with a diagnosis of Pre diabetes. Another medical term for this condition is 'Impaired Fasting Glucose' or IFG. It is one of the several risk factors for the development of diabetes.
So you need to eat healthy and participate in adequate physical activity, in order to lose 5 to 10% body weight (if you are overweight.obese) to prevent this condition from progressing to diabetes.
The C peptide and Insulin levels appear clearly high. However there is no published evidence based guideline that recommends testing for these two if the blood glucose levels are not low. These tests are most useful when evaluating an individual who is experiencing low blood glucoses despite not being on any medications that are known to lower glucoses below normal (such as Insulin and some types of diabetes pills).

Hi thanks for your advice. Please note that I am not taking any medicine for diebetes still my post meal BG is on lower side. Some time it's goes upto 70 also. But my fasting BG is in the range of 95 - 105. My doctor is advising to take Metaformin. Saying that I am insulin resistance. Please advice should I start the same and also advice if this med is habit forming. Thanks.

1 Normal blood glucose levels after meals are 70 to 139 mg/dl. So your levels are still within normal range

2 Because you are not on any medication for diabetes, it is unlikely you can have a true episode of hypoglycemia. If your glucose drops below 70, then schedule an appointment with an Endocrinologist to investigate potential causes of this.

3 Metformin although not approved for the treatment of Pre diabetes, has been studied in individuals with this condition.
Pre diabetes (which is what you have) just like type 2 diabetes is associated with Insulin resistance. Metformin is approved for the treatment of type 2 diabetes to improve insulin resistance.
When I see someone like you in my practice, I advise patients to try lifestyle modification first for 3 months before considering metformin.
