Suggest Treatment For Severe Abdominal Infection While On Venlafaxine

Question: Dear sir,
I very frequent suffer from abodomen infection.In last two month it has been 04 times I suffered from abodemen infection.I used to take o2 during infection as advised by physician. Last time I also taken saline through vene.
What tests are recommended? .I also take more non-veg products like chicken; red meat & fish.
I am also taking venlafaxine 150 mg for depression & tapering it slowly.
I am afraid of weak immune system .

I can understand your concern on GI symptoms.
1) What other abdominal symptoms you have than diarrhea?
2) Repetitive loose motion require detailed evaluation to confirm the cause. For this you might require consultation with gastroenterologist, blood tests for white cell count, stool examination, colonoscopy, USG abdomen.
3) Your treatment is effective to relieve symptoms of diarrhea but without knowing exact cause, it is not possible to provide cause-specific curative treatment and hence there will be high chance of re-development of symptoms.
4) Different causes for repetitive diarrhea are irritable bowel syndrome, inflammatory bowel disease, food poisoning.

Some reports have been uploaded based on advise of my physician.SGPT is on higher side.he advised to take oil & ghee free diets .Non veg diet to be stopped & advised adliv fort.

I apologize for late reply.
1) Yes, fat-free diet is must based on elevated SGPT report. This might also cause fatty liver i.e. fat deposition in liver tissue which can be detected on USG abdomen. If you have not done in last 3 months then it is recommended to undergo USG abdomen based on elevated SGPT report.
2) Total white cell count is also elevated, suggesting presence of infection anywhere in body. Intestinal symptoms + high white cell count suggest possibility of intestinal infection and warrants to undergo stool examination (routine and culture) to detect specific infection. If you would have persistent symptoms with negative stool report then it is necessary for you to have a colonoscopy check to rule out mucosal diseases of intestine.

Your stool report suggests non-specific intestinal bacterial infection for which empirical antibiotic treatment would be sufficient.

Yes, treatment as started by your physician is appropriate for your symptoms. You can ask him/her to add anti-colic medication to relieve from occasional colicky type of abdominal pain associated with intestinal infection.

USG abdomen is not necessary for stool infection. But as you mentioned about high SGOT level, USG abdomen should be done to rule out fatty liver (This will just confirm the condition, it will not impact on treatment. So it is not urgent to do USG abdomen)

1) There is bacteria present in your stool culture report but this type of bacteria is normal in intestine without any harm. You should continue antibiotics only if you have symptoms i.e. loose stool, lower abdominal pain.

2) USG report is not required for your present symptom or for decision making on present treatment.

I refer to USG abdomen on suspicion of fatty liver due to presence of elevated liver enzymes.

3) You can plan for USG abdomen later on if USG abdomen was not done recently in last 3-4 months.

I saw your reports.
1) There is no stomach infection on USG report not in stool report.
2) E.Coli bacteria is present in stool but this is normal bacteria present in human intestine.
3) Your treatment is correct and must continue it. Apart from it, you must take following precautions - fat-free diet, high protein-diet, intermittent fasting, regular exercise, avoidance of alcohol, use of omega-3 fatty acid containing oil for food preparation.
4) In some patients Tab Rifagut also causes loose stool but not to worry for it.

Thank for replying the Queries.
I would like to ask:-
1) USG report says '' concretion in lower calyx of left kidney''.is it sign of renal stone.(Please refer USG report) already uploaded.
2) Doctor says if you feel problem we will proceed for colonoscopy.Is there any requirement of colonoscopy.At present i am taking Tab rifagut 550mg two times a day for five days Tab velgut three times a day for five days as advised by doctor.Some time i feel tendency of passing stool.
3) Can i take rice,curd,milk,chapatti,egg.
4) Precautions to be taken to avoid recurrent stomach infection & renal stone.
5) i also take venix xr 75 mg & Betacap tr 40 mg for depression/GAD since 5 years .Is its effect .

1) Renal concretion means deposition of salts as a raw material for stone formation.
2) At present there is no need for colonoscopy. You should continue treatment for a duration. If symptoms persist then can plan for colonoscopy.
3) Curd is good. If milk and rice aggravates diarrhea then you should avoid. You can take eggs
4) For renal stone - avoid dehydration, drink minimum 2.5 lit water daily. For stomach infection, properly cooked diet is must. Avoid unhygienic raw food.
5) You can continue your anti-depression medications.

Your repeat repeat SGPT level is slightly elevated. You should take care to avoid high fat diet, taking high protein diet, doing regular exercise and intermittent fasting.
There is no need for uric acid level examination for concretion in kidney. You should take care of drinking adequate water daily to avoid dehydration (dehydration cause concentrated urine formation which is responsible for stone formation).

I have repeated LFT test due to feeling of uncomfortable in stomach like passing of stool but I do stool maxm 2 times in a day.
My SGPT level XXXXXXX from 79.83 to 111 & shot level is 46.5.new report is uploaded
May please advise what to do ?.
Although between the two test I have eaten very less non veg foods.But I am in tension regarding any serious viral disease in liver.
I also taken pain killers like ibugseic plus a lot due to body pain.some doctor is advising that pain killers also increases SGPT.

1) Recent elevation in SGPT warrants repeat USG of abdomen to rule out inflammation in liver (hepatitis).
2) Yes, higher dosages of ibugesic might cause elevation of liver enzymes in some patients and this will reduce spontaneously without any intervention over period of time.
3) About precautions, I would repeat which I mentioned to you as a precaution for fatty liver i.e. protein rich diet, fat-free diet, extra-carbohydrate containing diet (if not diabetic).

As advised by you I am taking less fatty acid & stopped completely non-veg foods.
I have taken a lot ibugseic plus & antibiotics which might have caused elevation of SGPT & SGOT.I hope it's effect will automatically subside.
I would like to ask is there any more test required to know the cause of problem.i will again do USG abdomen as advised.Is there need of antiviral medicine for liver.I have also started brisk walking & have started milk thistle ( Food supplements).
Is there any test for hepatitis.
Presently I am taking medicines:-
1) Adliv forte
2) Evion-400 mg
3) Liv-38 d-Patanjali product.
4) Milk thistle
Is there any more need of medication.
Some doctor is also suggesting to take corticosteroids.
Please advise.
