2. How the problem started ? Whether it was all of sudden or build up gradually?
3. How long he abused alcohol?
4. What amount of alcohol he used to take every day?
5. His last alcohol intake?
6. Whether he took any treatment while stopping alcohol? If yes details please
7. What symptoms he developed while stopping alcohol?
8. Any other substance history?
9. History of any other medical illness? Details if percent
10. Details of any psychiatric illness?
11. What are the symptoms of depression as per you and other family members?
12. What are the symptoms of depression as per your father ?

Here are some answers to the questions you asked.
1. I have had depression on and off for 20 years.
2. It has been a gradual build up. Started heavily drinking around 20.
3. Started drinking at 15. Reached a peak at 25. Stopped at 27 after a spell in a clinic.
4. 9 per cent alcohol beer. 10-15 cans.
5. Stopped since 27. Never relapsed.
6. First took depression pills to try to stop but then got addicted to the pills. Stopped all medicines after 27.
7. After stopping alcohol- couldn't stop drinking coke, eating chocolate all the time. Binge eating sweets, cakes etc. drinking large quantities of Red Bull etc. Taking vitamin tablets in huge quantities such as Maca, Zinc etc.
8.No other substance history. No smoking or drugs.
9.No other medical illnesses.
10. No other psychiatric illness.
11. Main symptoms are not wanting work. Feeling sleepy lazy, etc. Not interested in anything. Do anything to avoid working. Watching internet TV etc. Doing large amounts of exercise, shopping etc. to avoid working. Also telling non stop lies to my parent to get money from them to pay for my lifestyle so I don't have to work.
12. Other symptoms- I get angry with the kids easily. Often have fights with my wife. Any generally do this as well to avoid working, since the arguing takes up so much time.

Thanks for reverting back to me.
I am glad to note that you decided to respond and have a great will power to do which is intended. The two instances which is know to me is never to relapse and never to work.
Fortunately the former was positive behaviour but the latter is considered negative behaviour and it seems that you developed a sense of personality which thinks that I can not work.
I feel that whatever you state, it will not be wrong to say that you have immense capacity to carry out activities despite the presence of depression (large amount of exercise, shopping).
Being a psychiatrist I feel there is something which is preventing you from carrying out desired work which is not mentioned anywhere in your post. This could be childhood experiences, past bad experiences while working, homesickness, fear of losing family and many others.
I am of the opinion that rather than searching medications which can help you, it will be more appropriate to go for psychotherapy. Considering available information I feel cognitive behaviour therapy will be the best option followed by psychodynamic psychotherapy.

I understand about what you are saying locally.
My situation is a little unusual. I am British but I have been living in Japan for 20 years. When I went to a clinic some 15 years ago I went back to the U.K. Unfortunately I can't go back to the UK. anymore.
I do speak Japanese but I would prefer to get some kind of therapy in English. Is there anyone you know in XXXXXXX who is really good, who I get therapy with online or by Skype or some other way?
