In my experience in the E.R, i usually administer a single shot of I.M diclofenac sodium for renal colic that is confirmed clinically in the presence of a stone in the urinary tract on imaging. Depending upon the response ranitidine in combo with drotaverine is given. If the pain still persists then spasfon , spasmex and liometacin are resorted to. Lornoxicam and tramdol are our options for severe intractable pain. However these are complicated decisions to be made only in the setting of a hospital. You can take OTC hyosine butylbromide for the pain and go to an E.R near you where proper assessment can be done based on clinical findings.

The stone is quiet large and would not be passed spontaneously. Most likely it would require lithotripsy or minimal invasive surgery depending upon its location.
